    Job Description

    • 15 patients per day
    • Full-time position: 5 days per week, Monday through Friday
    • Compensation and benefits: our compensation is fee for service (FFS); expected compensation is estimated salary range of $132,184 to $156,250 annually with no cap on productivity income potential - this full-time role is benefit eligible

    Key Responsibilities

    • Perform primary care in a post-acute setting for short-term and long-term patients
    • Manage chronic disease, diagnose illnesses and medical conditions, and implement plan of care
    • Order and interpret lab results
    • Prescribe and monitor medications
    • Consult with physicians and coordinate patient care with other healthcare practitioners
    • Educate patients and their caregivers on coping skills to handle chronic and age-related conditions
    • Build relationships with your patients, patients' family members, and facility staff to facilitate effective treatment delivery
    • Promote healthy lifestyle changes

    Requirements

    • A minimum of 1 year experience as a nurse practitioner or physician assistant in a primary care setting required
    • Post-acute care experience a plus, but not required
    • Active and unrestricted New York NP or PA license required
    • Active and unrestricted DEA with prescriptive authority required
