    Nurse Practitioner or Physician Assistant

    In this role, you will be responsible for driving high quality, safe, and cost-effective care of patients in our local post-acute care communities. You will work with an interdisciplinary team rounding on all patients as medically necessary as required by CMS. This includes but is not limited to H&P, admission, progress, transfer, discharge, and acute/follow up. You will also work with the facility to transfer short-term rehab patients to the level of care most appropriate for their medical needs; including returning to home, independent living centers, assisted living, and/or memory care.

    Our NP/PAs work closely with hospice, palliative care, and home care agency to ensure a safe transition home. Being action-oriented and having a "can do" attitude with a high standard of quality and safety in care is a top priority for our teams to help build a strong reputation in our community to support our patients.

    Job Description

    • Drive high quality, safe, and cost-effective care of patients in local post-acute care communities

    • Conduct H&P, admission, progress, transfer, discharge, and acute/follow up
    • Collaborate with the facility to transfer short-term rehab patients to appropriate levels of care

    Compensation and Benefits

    • Expected salary range of $100,020 to $118,025 annually with no cap on productivity income potential

    This full-time role is 4 days per week with day call weekend rotation. This opportunity is benefit eligible.
