    Advanced Practice Provider (APRN or PA-C)

    The Advanced Practice Provider (APRN or PA-C) is responsible for delivering high-quality primary care to patients with both acute and chronic conditions in a fast-paced clinical environment. The role focuses on providing efficient patient care, addressing preventive care gaps, and maintaining accurate documentation while supporting value-based care initiatives. The provider will work closely with Medical Assistants and clinical staff to ensure smooth patient flow, timely chart completion, and high standards of clinical performance.

    Job Description

    • Provide comprehensive primary care for patients with acute and chronic conditions.
    • Manage an average of 23-24 patients per day using structured 20-minute appointment templates.
    • Identify and address care gaps (HEDIS) and ensure accurate HCC/RAF coding during each patient visit.
    • Ensure patients receive appropriate preventive screenings and follow-up care.
    • Lead and coordinate with Medical Assistants and Scribes to prepare patient charts, review care gaps, and pend orders before visits.
    • Maintain accurate and timely EMR documentation, ensuring 95% of charts are signed and closed within 48 hours.
    • Follow scheduling templates to maintain consistent patient flow and clinic efficiency.
    • Deliver patient-centered care while supporting value-based care initiatives and quality metrics.
