Senior Care Physician

ThedaCare, Wisconsin's largest non-profit healthcare system, is seeking a Senior Care Physician to join our established team in Appleton. You will be responsible for working with PALTC patients in community living facilities, developing relationships while administering high-quality patient care.

Job Description

• Full-time (4-10s) or part-time options available with flexible schedules
• Location: Appleton
• The team consists of 5 physicians and 8 APCs
• 2-3 SNFs a day (patient schedule built within EPIC)
• On-call by phone only: 1:11
• Patient census: 8-12
• Support staff includes RNs and schedulers who help to coordinate provider visits, communicate with facilities, and triage phone calls
• 27 facilities total between skilled nursing and assisted living facilities
• Large opportunities for growth, such as Medical Director roles in the nursing homes that we serve
