    Associate Medical Director

    Tufts Medicine Care at Home is currently seeking an Associate Medical Director to join our hospice team in the greater Lowell area. This position will be primarily community-based with a 4-day per week schedule.

    Our care team includes physicians who are board certified in hospice and palliative care, nurse practitioners with extensive experience in pain and symptom management, and social workers who are knowledgeable about the physical, emotional, and spiritual issues that often accompany a serious or life-limiting illness.

    Job Description

    • Provide direct medical care, disease management, pain, and symptom management of hospice patients.
    • Conduct clinical and public education regarding hospice care.
    • Provide guidance to hospice nurse practitioners and nurses as they provide direct patient care.
    • Be an integral part of the interdisciplinary group and attend IDG meetings as scheduled, act as a medical resource for the team, guide the work of those who provide direct medical services (RN, NP, PA, LPN, CNA), and assure compliance with medical standards of care for hospice.
    • Certify that the patients meet the medical criteria for hospice admission.
    • Re-certifying patients, as appropriate, for continuation of the Medicare Hospice Benefit at the appropriate levels of care.
    • Assure physician representation and participation in the development of a patient's hospice plan of care.
    • In conjunction with the hospice team, participate in family meetings to educate patients and families on medical conditions and the pros and cons of treatments or medications.
    • Consult with patient PCPs as needed regarding eligibility for hospice care, pain, and symptom control. Complement attending physician care.

    Requirements

    • Board certification in family or internal medicine and hospice/palliative medicine or geriatrics preferred.
    • Current or ability to obtain medical licenses in Massachusetts and New Hampshire.
    • Experience in the care of terminally ill patients.
    • Current driver's license and auto insurance.
    • Demonstrated ability to work as a team player and function as a member of an interdisciplinary team.
