Wound Care Nursing Team Lead
The Wound Care Nursing Team Lead will be responsible for assisting in the supervision and coordination of all activities relating to the Wound Care Nursing Team. The Team Leader provides clinical coordination of care related to skin integrity and wound care, extending across CVMC and WDR and to the outside community. The Team Lead provides professional nursing care to newborn through end-of-life patients in varying states of health and illness through assessment, planning, implementation, and evaluation of the patient's health needs and goals. The Team Leader provides direct care to patients and families, as well as teaching and preparation for continuing care. The Team Lead is an experienced nurse who is the clinical expert and resource for WDR residents, ED, ACU, and CVMC inpatients, medical team staff, and the organizations at large. The Team Leader demonstrates clinical leadership skills through mentoring, providing educational programs for staff, and modeling excellent clinical and customer service. This Team Leader will conduct their clinical duties primarily at Woodridge Skilled Nursing & Rehabilitation and will have oversight of the nursing wound care team functioning at both Central Vermont Hospital and Woodridge Skilled Nursing and Rehabilitation.
Job Description

- Deliver professional nursing care to patients from newborn to end-of-life
- Conduct assessments, planning, implementation, and evaluation of patient health needs
- Provide direct care to patients and families, including teaching for continuing care
- Serve as a clinical expert and resource for various medical teams
- Demonstrate clinical leadership through mentoring and educational programs
Requirements
- BSN required
- Current RN licensure recognized by the State of Vermont required
- CWCN Certification (or within one year of hire) is required
- Appropriate experience in specific clinical area; varies by unit
