    Job Description

    • Provide prescribed medical treatment and personal care services to patients with diseases and injuries seeking treatment in the clinic, as directed by physician or mid-level provider.
    • Collaborate with the core PACT Team (Primary Care Provider, RN, and Medical Assistant) and expanded PACT Team including family/caregiver, VA, and community-based services involved in developing the patient care plan.
    • Evaluate veterans for mental health disorders and appropriately diagnose and treat mental health conditions.
    • Order and perform diagnostic tests and therapeutic procedures as related to mental health diagnoses and treatment and interpret test results for deviations from normal and provide follow-up care.
    • Prescribe medication and recommend dietary and activity programs.
    • Perform appropriate interval follow-up visits for necessary treatment of mental health diagnosis.
    • Refer patients to the VA medical specialist when indicated.
    • Counsel patients on the use of prescription medications and educate patients in relation to treatment plan.
    • Develop and implement patient management plans, assisting in the provision of continuity of care through the PCMM (Primary Care Management Module).
    • Evaluate patients' records from medical providers outside the VA and work with veterans utilizing rules set forth by the VA for co-managed care.
    • Conduct individual counseling/therapy.
    • Contribute to group therapy (i.e., PTSD groups; weight management; smoking cessation; grieving; and personal interactions).
    • Contribute to pain management plan as needed.
    • Involve veterans in planning and assuming responsibility for activities.
    • Refer veterans, when indicated, to community resources and other organizations.
