    Job Description

    • The Nurse Navigator (NN) is a licensed registered nurse with disease-specific clinical knowledge who offers individualized assistance to patients, families, and caregivers to help overcome barriers to care.
    • Utilizing the nursing process, a nurse navigator provides education and resources to facilitate informed decision-making and timely access to quality health and psychosocial care throughout all phases of the cancer continuum.
    • This role is both patient and provider facing with the ability to interface both in person, telephonically, and electronically though it is not a bed/chair side position.
    • The primary functions of this role are to promote timely access to care and compliance with the plan of care.
    • Key responsibilities include serving as a subject matter expert in the field of navigation, improving patient care timeliness and access to care, collaborating with program leaders and administration, providing relationship-based patient-centered care, and participating in growth and development activities with the navigation team.

    Requirements

    • Required Minimum Education: Associates Nursing or Diploma (Nurse) Nursing or Bachelors Nursing (Preferred)
    • Required Minimum License(s) and Certification(s):
      • ACLS - Advanced Cardiac Life Support or ARC-ALS - Amer Red Cross Advanced Life Support or ACLS-I - ACLS - Instructor or ACLS-P - ACLS-Provisional (180 Days) within 180 Days
      • BLS - Basic Life Support or ARC-BLS - Amer Red Cross Basic Life Support or BLS-I - Basic Life Support - Instructor
      • RN - Reg Nurse (Single State) or RN-COMPACT - RN - Multi-state Compact
    • Required Minimum Experience: Minimum 3 years nursing practice required and minimum 2 years nursing with disease-specific experience preferred; outpatient care or care coordination experience preferred.
