Our client is seeking a Family Nurse Practitioner (FNP) for an outpatient community health center in Willimantic, CT. Job Description

• 100% outpatient position, focused on seeing patients of the health center.
• Slow ramp up during training, but eventually a full panel considered 15-20 scheduled patients per day.
• Patients of all different demographics and socioeconomic status - work collaboratively with other providers.
• Evaluation, diagnosis, medication assessments and adjustment, patient education, and treatment planning.
• Hours: Monday-Friday, 8-5.
• FQHC/Community health - student loan forgiveness eligible.
• One location has a 40% Spanish population and prefers medical or fluency in Spanish.

Requirements

• CT State license, DEA, CSR (active or applied preferred) but not required.
• Bilingual language skills in Spanish preferred, but not required.
