Job Description

• Conducting thorough health assessments and diagnosing both acute and chronic conditions.
• Developing, coordinating, and managing individualized treatment plans.
• Prescribing medications and therapies in accordance with state and federal guidelines.
• Partnering with interdisciplinary teams to promote continuity of care and patient education.

Requirements

• Clinical leadership experience preferred.
• NY State License.
• Board Certified in Family Medicine.
• Current BCLS.
• Current DEA Certificate.

Compensation

• $130,000 - $140,000
