LPN/Intake Coordinator - Onsite Call Center - Night Shift

Join our team at Willow Creek Behavioral Health in Green Bay, Wisconsin, where we offer a supportive environment for our staff and patients. This position involves 12-hour shifts, including every other weekend, from 6 PM to 6:30 AM, with a starting pay of $26 per hour and up based on education and experience.

Job Description

• Provide a single point of entry for in-patient intake.
• Conduct phone triage, address requests for services, and perform patient callbacks while documenting all call activities.
• Answer inquiry calls about hospital services and bed availability.
• Perform pre-registration, collect pertinent data, verify insurance eligibility, and coordinate with the insurance company.
• Facilitate and coordinate assessment and/or intake to facility services when appropriate.
• Maintain communication between the organization and patient referrals and families.
• Conduct pre-admission screenings and schedule assessments.
• Work closely with the business office, nursing, and clinical staff as well as external parties to ensure the timely admission of patients.
• Refer inquiries to other agencies and community resources when not appropriate for facility assistance or admission.
• Provide information and referral services for internal and external customers.
• Present clinical information to a member of the medical staff for approval of intake.
• Provide crisis intervention services if needed.
• Verify insurance upon intake.
• Maintain all documentation involved with the admissions process.
• Provide ongoing communications with referral sources concerning the status of patients referred into the program; promote and provide education regarding available services.
• Provide clerical support to admissions, including collating admission folders, ensuring an adequate supply of pre-stuffed admission folders, auditing patient charts, and monitoring patients while in an internal waiting room.

Requirements

• LPN with one or more years of experience in healthcare admissions required, preferably in the mental health or substance use disorder field.
• Experience in patient assessment, referral, treatment planning, and communication with external review organizations or comparable entities.
• Knowledge of community resources.
• Outstanding interpersonal, interviewing, and assessment skills.
• Skill in telephone etiquette and paging procedures.

Licenses/Certifications

• CPR and de-escalation and restraint certification required (training available upon hire).
• First aid may be required based on state or facility requirements.

Supervisory Requirements

This position is an individual contributor role.
