Wise Medical Staffing is seeking an experienced Registered Nurse (RN) Case Manager for a 13-week travel contract in Asheville, NC. The RN Case Manager will play a vital role in care planning, discharge coordination, and ensuring quality outcomes.

Job Description

• Coordinate and manage individualized patient care plans
• Collaborate with physicians, nurses, and interdisciplinary healthcare teams
• Monitor patient progress and adjust care plans as needed
• Ensure accurate documentation and regulatory compliance
• Facilitate discharge planning and patient education
• Advocate for appropriate resource utilization and optimal patient outcomes

Requirements

• Active RN license (Compact license preferred)
• Minimum 2 years of Case Management experience required
• Current BLS certification
• Strong communication and interdisciplinary collaboration skills
• Excellent organizational and critical thinking abilities
• Proficiency with clinical documentation
