Requirements

• MD/DO.
• Unrestricted state medical license.
• Unrestricted DEA license or ability to obtain.
• OB/GYN BC/BE (ABOG, AOBOG).
• Completion of ACGME OBGYN residency program.
• Board certification required within 5 years of eligibility.
• BLS.
