As a Primary Care Physician, you will provide high-quality, outpatient care in a value-based care model, focusing on building long-term relationships with patients while driving positive health outcomes.

Job Description

• 100% outpatient primary care
• Patient population: seniors with 3-5 chronic/complex conditions (e.g., heart disease, diabetes, hypertension)
• Average of patients per day
• Patient panel capped at 650
• Dedicated care team including MA, scheduler, referral specialist, and full leadership support
