- Providers will collaborate closely with community OB physicians and hospital staff to ensure continuity of care, strong communication, and high-quality patient outcomes.

**Requirements**

- Board Certified in OB/GYN
- Minimum of one year of recent clinical experience
- Active Ohio medical license preferred or IMLC eligibility/in process
- Completed prescreen questionnaire
- Updated CV in MM/YYYY format with all gaps explained
- Current Self-Query NPDB report (within 30 days)
- Ability to provide full credentialing and submission documentation at time of name clear
