Join our PrimeCare team as we seek a board-certified Nurse Practitioner or Physician Assistant to help build and grow a new practice located in Northern Onondaga County. This full-time, permanent position is based in Phoenix, NY, just 35 minutes from Syracuse, and is scheduled to open in Winter 2026.

**Job Description**

- Work as a physician extender in a family medicine practice that delivers comprehensive patient care in an outpatient setting.
- Collaborate with physicians to assess, diagnose, and develop a plan of care in accordance with practice protocol.
- Obtain complete medical histories, perform physical examinations, and order appropriate diagnostic tests.
- Make referrals for specialty consultations and care.
- Spend time in a clinic setting seeing new and established patients with the support of administration and an experienced physician team.
- Ideal candidates will have 5+ years of experience working in an outpatient primary care setting.
