**Job Description**

- Ideal candidates are ambitious, self-starters with plenty of volume and an interest in high volume, high compensation.
- Daily clinic volume of 35-40 patients.
- Perform 3-5 procedures daily in hospital/OBL.
- No special equipment or EMR required.

**Compensation and Benefits**

- Base compensation: 
  - 1st year: $650,000 
  - 2nd year: $600,000 (to receive full base salary, the physician must produce $1 million in gross revenue for the company)
  - 3rd year: $550,000 (same revenue requirement for full salary and consideration for a 3% ownership in the OBL)
- Sign-on bonus of $50,000.
- Relocation bonus of $12,000.
- CME, license fees, and full health coverage.

**Vacation**

**Schedule and Call Requirements**
