About the Position POSITION: Registered Nurse - Substance Use Disorder care Coordinator EMPLOYMENT TYPE: .8 FTE (64 hours in a pay period) TEMP to last aprx. 4 months WORK SCHEDULE: Days Job Summary: This role reports to clinic nursing management and is responsible for ensuring competent, compassionate, and individualized nursing care for patients with substance use disorder (SUD), with some focus on addressing neonatal abstinence syndrome (NAS). This role collaborates with all members of the care team to ensure comprehensive and coordinated patient care and seamless care transitions. The SUD Care Coordinator RN will be an ongoing resource between patients and providers for pain care and medication assisted therapy (MAT) management. This position is a part of the Health Resource Services Administration (HRSA) Rural Communities Opioid Response Program (RCORP) grant where funding will be used to strengthen and expand SUD/opioid use disorder (OUD) prevention, treatment, and recovery service delivery in our community. Grand Itasca's RCORP project will: (1) Reduce structural and systems-level barriers within the rural target service area to (a) Increase access to behavioral health care, specifically SUD/OUD, services for rural pregnant and postpartum persons and their families; and (b) Address community risk factors and social determinants of health. (2) Strengthen the quality and sustainability of behavioral health care services for rural pregnant and postpartum persons and their families in the service area by implementing coordinated, evidence-based, family-centered SUD/OUD and other services. - This role reports to clinic nursing management and is responsible for ensuring competent, compassionate, and individualized nursing care for patients with substance use disorder (SUD), with some focus on addressing neonatal abstinence syndrome (NAS).
- Collaborates with all members of the care team to ensure comprehensive and coordinated patient care and seamless care transitions.
- Acts as an ongoing resource between patients and providers for pain care and medication-assisted therapy (MAT) management.
- Part of the Health Resource Services Administration (HRSA) Rural Communities Opioid Response Program (RCORP) grant aimed at strengthening and expanding SUD/opioid use disorder (OUD) prevention, treatment, and recovery service delivery in our community.

**Job Duties**

- Follows the RN process for nursing assessment, diagnosis, planning, implementation of care, and evaluation of patient progress to function in a full scope RN role to support providers in the clinic.
- Works collaboratively with providers to triage patients, identify patients' level of care needs, provide care coordination, and required interventions using assessment, critical reasoning, and clinical judgment.
- Assesses and medically manages intoxication and withdrawal from substances using standardized screening and assessment tools/scales.
- Manages, understands, and reviews with patients opiate and opioid medication contracts for patients with SUD.
- Provides health education to facilitate patient treatment, including but not limited to medications for SUD and PrEP (Pre-exposure prophylaxis), safer drug use, and prevention of infections.
- Provides and administers outpatient pharmacotherapy for SUD, including opioid antagonist or agonist therapy such as naltrexone, buprenorphine, and methadone.
- Works with the pharmacy and the outpatient team to obtain medication orders, prescriptions, refills, and communication as needed.
- Consults with members of the treatment team and other treatment professionals to devise an individualized plan for optimal psychosocial treatment for individual patients.
- Reviews patients' charts and follows up with patients and members of the health care team as needed to ensure that patients are current with all necessary medications, tests, screenings, and follow-up care.
- Tracks and follows up on critical referrals and test results with patient and care team.
- Documents nursing assessment, interventions, plan, and outcome in the patient record in a timely manner.
- Evaluates effectiveness of care and interventions for patients with SUD concerns in collaboration with the health care team.
- Communicates with members of the health care team, including inpatient and outpatient teams, to ensure continuity in care transitions.
- Knows the key staff available to help patients with financial and emotional support; arranges for patients to meet with these resources if needed.
