- Plan, direct, monitor, and evaluate daily operations to ensure that the community-based programs provide high-quality services, consistent with contracting specifications and in accordance with the Quality Management Plan (QMP).
- Participate in professional outreach activities to further develop community partnerships, potential referral sources, and the growth of the programs.
- Ensure compliance and work in conjunction with standards pertaining to credentialing agencies such as The Joint Commission and Department of Public Health.
- Work in collaboration with the care teams, identify appropriate level of care, and provide additional professional supports for patients as needed.

- Education: Master's degree in social work or counseling.
- Experience: Two (2) years of management or supervisory experience. Two (2) years of experience in behavioral healthcare and/or substance use disorder treatment. Two (2) years of experience in community-based care management or hospital-based social work preferred. Two (2) years of experience in community outreach preferred.
- Skills: Management or supervisory skills required. Knowledge of patient treatment plans/care plans.
