**Medication History Technician**

The Medication History Technician, under the supervision of a licensed Pharmacist and in accordance with all State and Federal laws, will perform medication history assessments for patients being admitted to the hospital, providing complete information regarding each medication identified (i.e., drug, dose, route, frequency, indication, last dose taken). The technician will verify patient allergy information and ensure accuracy of the prior to admission (PTA) medication list.
