**Salary/Hours**

Starting salary is $79,480 - $90,119 annually with scheduled annual increases that can result in a salary of $111,407 per year. This is a full-time position requiring 40 hours per week.

**Family Nurse Practitioner**

Join our team as a Family Nurse Practitioner in the home health sector, providing essential in-home health and wellness assessments for health insurance plans.

**Job Description**

- Conduct in-home health and wellness assessments, including history and physical evaluations.
- Review medications and dosages, perform physical exams, check blood pressure, conduct finger stick tests, obtain urine samples, and provide fecal kits for patients to mail in.
- Complete 5 to 8 assessments per day, with each assessment lasting approximately 45-75 minutes.
- Work with geriatric patients.
