**Job Description**

**Requirements**

- Board Certified or Board Eligible in Family Practice required.
- Documentation of completion of education as Medical Doctor (MD) or Doctor of Osteopathy (DO). If medical education was obtained outside of the U.S., documentation from the ECFMG is required.
- Current and unrestricted Florida medical license required.
- Current and clear DEA registration.
- Current BLS/CPR certification.
