- Completed approved Doctor of Medicine or Doctor of Osteopathy residency program in a specialty such as Family Medicine or Internal Medicine resulting in board eligibility to board certification.
- Licensed as a physician to practice medicine in the State of West Virginia.
- Active WV DEA certificate without restrictions.
- Current BLS and ACLS required.
- Credentialed and privileged as a member in good standing of the Medical Staff of Mon Health Medical Center.
