Hybrid Collaborative Care Clinician located in Olympia Kaiser Permanente clinic. Minimum of 2 days on-site at assigned primary care clinic location. Job Summary: Under supervision of a Licensed Clinical Independent Social Worker, Licensed Marriage Family Therapist or a Licensed Professional Clinical Counselor, provides a wide range of clinical psychosocial services for adult and/or child members in a primary or specialty care, mental health or home health setting which may include: evaluation of psychosocial assets and deficits, short term counseling, education, advocacy, advanced care planning, and resource brokering with caregiving and/or other resources to enable and/or maintain the greatest level of independence; mental health and addiction assessment, triage, diagnosis, treatment and/or crisis intervention services; support and consultation to care team in managing patients with a broad range of psychosocial related needs. Under supervision, may collaborate with treating primary or specialty care providers and with psychiatric, addiction medicine, and allied health professional team members to plan and direct each individual patients treatment program. Essential Responsibilities: <ul> <li>Confers and consults with supervisor regarding members progress and treatment and may consult with psychiatrist or other care team providers as well, if involved in members care.</li> <li>May provide appropriate support to members family, including explanation of treatment, instructions in how to support treatment and interventions to increase acceptance of and adherence to treatment, at members or surrogate decision makers request.</li> <li>Utilizes resources of public and private agencies and community organizations to meet the needs of the members treatment to include referral of the member and/or members family to external resources, as appropriate.</li> <li>Participates in departmental program development, implementation and evaluation.</li> <li>Reports safety concerns to mandated reporting agencies.</li> <li>Develops, implements, coordinates, and evaluates plan of care and/or clinical treatment programs for the diagnosis, treatment, and/or referral of Health Plan members with acute or chronic mental or addiction illness. **Hybrid Collaborative Care Clinician**

Join our dynamic team as a Hybrid Collaborative Care Clinician located in the Olympia Kaiser Permanente clinic. This position requires a minimum of 2 days on-site at the assigned primary care clinic location.

**Job Description**

- Under supervision of a Licensed Clinical Independent Social Worker, Licensed Marriage Family Therapist, or a Licensed Professional Clinical Counselor, provide a wide range of clinical psychosocial services for adult and/or child members in a primary or specialty care, mental health, or home health setting.
- Evaluate psychosocial assets and deficits, provide short-term counseling, education, advocacy, advanced care planning, and resource brokering to enable and/or maintain the greatest level of independence.
- Conduct mental health and addiction assessment, triage, diagnosis, treatment, and/or crisis intervention services.
- Support and consult with the care team in managing patients with a broad range of psychosocial-related needs.
- Collaborate with treating primary or specialty care providers and with psychiatric, addiction medicine, and allied health professional team members to plan and direct each individual patient's treatment program.
- Confers and consults with supervisor regarding members' progress and treatment and may consult with psychiatrist or other care team providers as well if involved in members' care.
- Provide appropriate support to members' families, including explanation of treatment and instructions on how to support treatment and interventions to increase acceptance of and adherence to treatment.
- Utilize resources of public and private agencies and community organizations to meet the needs of the members' treatment.
- Participate in departmental program development, implementation, and evaluation.
- Report safety concerns to mandated reporting agencies.
- Develop, implement, coordinate, and evaluate plans of care and/or clinical treatment programs for the diagnosis, treatment, and/or referral of Health Plan members with acute or chronic mental or addiction illness.
- Instruct and counsel patients and their families regarding compliance with the plan of care.
- Administer specialized therapeutic interventions as appropriate.
- Complete all documentation of the members' treatment and progress in accordance with Kaiser Permanente, state, and NCQA regulations.
