**Job Description**

- Hours are Monday & Wednesday 7:00 am-5:00 pm, Tuesday & Thursday 8:00 am-6:00 pm, both with a 60-minute lunch break, and Friday 8:00 am-12:00 pm.

**Requirements**

- Provider must be Board Certified.
- Must have 2+ years of clinical experience in a primary care specialty to qualify for this assignment.
