- Average of 5 patients per day; manage chronic disease states of the geriatric population
- Work with care plans within a group that includes Nurse Practitioners, Internal Medicine Doctors, Physical Therapists, Occupational Therapists, Dieticians, and other specialties for each patient
- Manage dialysis treatment setup and coordinate all care for these patients daily
- EMR is E-Clinical Works (training available)

**Requirements**

- Board certification in Internal Medicine or Family Medicine
- California medical license
- DEA and BLS certifications
- Geriatric experience
- Experience with PACE program or team-based practice
- Two references from DO/MD
