**Job Description**

- Manage a volume of approximately 12-15 outpatient visits per day, consisting of established patient visits and answering clinical questions.
- No surgeries or procedures required.
- Provide care for 24 patients with illness related to the practice of Hematology within the last 12 months.

**Requirements**

- All medical schools must be on the California Approved List.
- All residencies and fellowships must be ACGME approved.
- No large malpractice issues.
- Clean NPDB.
- Three professional references that have worked with the provider within the last 18 months.
- Active Michigan license (IMLC accepted), DEA, and Cs3.
- Case/patient logs showing 24 Hematology patients within the last 12 months.
- Board certified or board eligible within the last 5 years.
