**Family or Internal Medicine Physician**

We are seeking a Family or Internal Medicine Physician to join our innovative care team in West Point, GA. Deliver whole-person care in 30- to 60-minute appointments, supported by a health coach and care team, focusing on high engagement, managed care, and culture transformation to reduce costly service utilization and improve overall health outcomes.

**Job Description**

- See 10-15 patients daily in longer, meaningful appointments.
- Collaborative, team-based approach with health coaching support.
- Focus on proactive, value-based care.

**Compensation and Benefits**

- Salary: $238,000 - $350,000 + bonus eligibility.

**Requirements**

- MD or DO degree with completed Family Medicine or Internal Medicine residency (Family Medicine preferred).
- Board Certified by ABMS (ABFM/ABIM) or AOA.
- At least 3 years of primary care experience.
- Current, valid medical license and DEA number.
- Proficiency with EMR systems (NextGen preferred), coding, and Medicare Advantage/HCC documentation.
- Experience with Google Workspace and team-based care.
