Completes regulatory required assessments within specified timeframes. Completes medication reconciliation at specified timeframes.Qualifications: 1-2 years outpatient rehabilitation setting. At least two (2) years of physical therapy experience highly preferred. One (1) year experience in home health or other health setting with geriatric emphasis. Current license in Physical Therapy or ability to obtain prior to hire. American Heart Association HealthCare Provider BLS preferred upon hire, and required within 90 days of hire.
