Behavioral Health Director

Mountain Community Health Partnership (MCHP) is seeking a full time Advanced Practice Provider (Physican Assistant or Nurse Practitioner) to join our two middle school School-Based Health Centers (SBHCs) in Yancey County, NC. The care team at these sites include an experienced RN and a Clinical Social Worker who specialized in children and adolescents and is supported by a Family Medicine Physician with board certification in adolescent health. MCHP is the largest provider of pediatric (and adult) care in our community and our SBHCs are an essential part of the care we provide. Patient visits are a combination of acute car, well adolescent care and management of chronic health issues for both students and staff both in person and via telehealth. **Advanced Practice Provider**

Mountain Community Health Partnership (MCHP) is seeking a full-time Advanced Practice Provider (Physician Assistant or Nurse Practitioner) to join our two middle school School-Based Health Centers (SBHCs) in Yancey County, NC. The care team at these sites includes an experienced RN and a Clinical Social Worker who specializes in children and adolescents and is supported by a Family Medicine Physician with board certification in adolescent health.

MCHP is the largest provider of pediatric (and adult) care in our community, and our SBHCs are an essential part of the care we provide. Patient visits are a combination of acute care, well adolescent care, and management of chronic health issues for both students and staff, both in person and via telehealth. During the summer and school vacations, the Advanced Practice Provider will rotate through our four other practices, seeing patients from the SBHCs and across our patient panel.

The ideal candidate will enjoy the complexity of adolescent health care, work well in a multidisciplinary team, and recognize the clear relationship between health and academic achievement.
