The Advanced Practitioner at Monogram Health is a key member of an integrated Care Team which includes a Registered Nurse and a Social Worker. **Job Description**

- Deliver personalized compassionate medical care to individuals mainly with CKD and/or ESRD/ESKD.
- Care for patients, maintaining accurate and current patient records and scheduling, and administering follow-up appointments as required.
- Strive to change the narrative in healthcare by putting patients and their quality of life at the forefront.
- Conduct assessments, including comprehensive annual wellness exams on patients both in the patients' home and in the virtual environment.
- Counsel and educate patients and families about benefits and programs available to help them live healthier lives.
- Document items such as appropriate chief complaint, all applicable diagnosis, past medical, family, and social history, review of systems, examinations, medications, allergies, assessment, and plan.
- Coordinate care with primary care providers, specialists, and appropriate ancillary services.
- Complete all documentation and paperwork in a timely manner.
- Maintain quality of care standards as defined by the practice.
- Deliver evidence-based, timely care in a manner that reduces avoidable hospitalizations, maximizes quality of life, and puts patient health and satisfaction first.
- Prescribe medications, order tests, and collaborate with the patient's Monogram physician.
- Facilitate closing gaps in care by educating patients about preventive monitoring and working with physician practices to schedule diagnostic testing.
- Assist patients with enrolling to access educational videos.
- Participate in integrated care team meetings.
- Make assessments of patient's health status and develop treatment plans.
- Implement plans consistent with appropriate plans of care.
- Follow up and evaluate patient's status.
- Other duties as assigned.

**Requirements**

- Bilingual (English/Spanish) highly preferred.
- Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license.
- Board certified for appropriate licensure (NP: ANCC/AANP; PA: NCCPA).
- Current and unrestricted DEA certificate.
- Ability to work without direct supervision and practice autonomously.
- Access to transportation, a valid driver's license, and car insurance.
- Proficient with medical instruments and equipment required by the work.
- Knowledge of computer-based data management programs and information systems, as well as medical records and point-of-interview technology.
- Ability to communicate effectively in verbal and written form with retail and medical partners at various levels, patients, family members, physicians, and representatives of the community.
- Sound understanding of all federal and state regulations including HIPAA and OSHA.
- 2 or more years of direct patient care required.
- Managed Care/IPA/Health Plan experience.
- Experience conducting annual wellness visits or similar comprehensive visits virtually or in the home.

The physician must have an active GA license, be certified (BE/BC) as a hospitalist, have a Clean DEA & NPDB (no active suits or unresolved cases), and have a minimum of one year experience. The physician must also have ACLS, be able to intubate, and sign off on intubations for credentialing. The provider is typically required to do admits and rounding. The following dates of coverage are needed in these communities: Madison, GA: 9/9 - 9/11 9/29 - 10/12 11/21 - 11/27 (Thanksgiving) 12/20 - 12/26 (Christmas) 12/30 - 1/2/23 (New Years) Provider must be able to take a minimum of one full 4 to 7 day period. All shift are 24 hours, typically 12-15 hours shifts plus night call. Shift times are generally 12pm - 12pm. Orientation is always on a weekday, and is typically 4 hours. If the shift falls on a weekend, orientation will be scheduled sometime Monday through Friday between 8am & 5pm. If the shift falls on a weekday, the provider must arrive on the first day at 8am to complete orientation from 8am-noon. Morning rounds typically begin between 7am and 8am.
