Position Overview The CINQCARE Care at Home program is designed to provide additional support to our patients by providing an integrated level of equitable value-based medical care and social support in the comfort of where our members call home. This program is offered to eligible health plan patients with the primary goal of mitigating adverse health complications, unmanaged disease progression and ultimately avoid unnecessary hospitalization that can occur when timely clinical interventions are not provided or are not accessible. In this role, you'll collaborate closely with a multi-disciplinary clinical team to deliver high-quality, personalized care in both a home-based and telehealth setting. Work in partnership with patients in the prevention of illness, promotion of health, and management of patient's adjustment to chronic health problems. Provide back up call to an overnight, holiday and weekend nurse line for clinical escalations in multiple states. Licensing requirement: Willingness to obtain and maintain Nurse Practitioner licensure in multiple states to support on-call backup coverage, with all associated licensing costs covered by the company.
