- Under the direction of the Director of Nursing/LTC Clinical Coordinator, responsible for the planning and coordination of direct resident care services.
- Serve as a core member of the interdisciplinary client care team with the authority to assess, plan, evaluate, and teach in patient situations.
- Perform professional nursing care to residents according to needs and supervise nursing activities of the facility.
- Conduct daily rounds and communicate with appropriate team members during the shift.
- Contact providers to establish good working relationships and ongoing information exchange.
- Administer routine medications, document, and make decisions about the use of PRN medications. Obtain specimens for lab tests as ordered.
- Complete documentation in a timely manner, including all medications and treatments administered, and participate in the development of interdisciplinary plans of care.
- Participate in interdisciplinary team meetings and coordinate additional meetings as necessary to address crisis matters and provide support for team members.
- Maintain established departmental policies and procedures, objectives, quality improvement, environmental safety, and infection control standards.
- Initiate resident referrals, transfers, and discharges.
- Ensure that assigned CNAs/CMAs understand each resident's plan of care and assist in training and education of nursing and nursing support staff.
- Maintain supplies and equipment in adequate numbers and working order following infection control policies.
- Provide resident and family education by direct instruction or utilizing interdisciplinary team members or appropriate community resources.

**Requirements**

- Licensed Practical Nurse (LPN) - Board of Nursing Active license in the state of practice upon hire.
- Minimum of 3-5 years experience.
