**Job Description**

- Deliver high-quality, safe, and cost-effective care in post-acute settings
- Provide inpatient rounding in:
  - Skilled Nursing Facilities (SNF)
  - Inpatient Rehabilitation Facilities (IRF)
  - Long-Term Care Hospitals (LTACH)
- Collaborate with interdisciplinary care teams in accordance with CMS guidelines
- Coordinate care transitions with hospice, palliative care, and home health agencies
- Ensure timely documentation and billing in compliance with Medicare guidelines using gEHRimed EMR

**Requirements**

- Board Certified: FNP, AGACNP, ACNP, AGPCNP, or PA-C
