- The APC will ensure the delivery of high-quality clinical and home-based patient care while promoting optimal health outcomes and providing cost-effective solutions that adhere to Alignment Health's policies, as well as state and federal regulations.
- You will play a vital role in assessing, developing, and coordinating care options tailored to the needs of our members, who are typically 65 and older. With home and virtual visits ranging from 45 to 60 minutes, you will have the opportunity to provide focused, compassionate care, whether it’s an initial visit, an annual check-up, or when additional support is needed to address medical and social challenges.

**Requirements**

- Must be able to perform essential physical functions, including talking or hearing, standing, walking, sitting, using hands to handle objects, tools, or controls; and reaching with hands and arms.
- Frequently lift and/or move up to 20 pounds. Specific vision abilities required include close vision and the ability to adjust focus.

**Benefits**

- Approximate salary range: $150,000 to $160,000 yearly
- Yearly bonus up to 7.5% of annual base salary
