- Board Certified, ACOG certified.

- Actively delivering babies (must submit case logs for the past 24 months).
- Case logs from the last two years (must be from a hospital, not an estimate from the provider) required in submission.
- No recent or open malpractice cases.
- Include disciplinary actions, if any.
- Prefer candidates within driving distance.
