Provides instruction to the patient/caregiver and revises the treatment plan as appropriate. Maintains communication regarding patients with the case manager and other disciplines via care conferences or other appropriate methods. Reports acute medical problems and condition changes to the primary care provider. Provides supervision to Physical Therapy Assistants, Home Health Aides, and students in the clinical setting. Completes regulatory required assessments within specified timeframes. Completes medication reconciliation at specified timeframes.Qualifications: 1-2 years outpatient rehabilitation setting. At least two (2) years of physical therapy experience highly preferred. One (1) year experience in home health or other health setting with geriatric emphasis. Current license in Physical Therapy or ability to obtain prior to hire. American Heart Association HealthCare Provider BLS preferred upon hire, and required within 90 days of hire. Current Driver s license Excellent customer service skills Ability to work without direct supervision. Provides care appropriate to the age of patients served on the job.
