Triages status changes in participants and facilitates appropriate management of identified problems. Provides assistance and oversight to PACE medical staff in the scope of their duties as needed. TREATMENT Provides preventive health care and health promotion for participants. Treats and manages acute chronic illnesses in the health center, nursing home, group home, or participants' homes. Regulates and adjusts medications as needed and in a cost-effective manner. Facilitates medical specialist consultation as needed and oversees participant's use of medical specialists and inpatient care. Educates participants and families regarding health maintenance and chronic medical conditions. Coordinates participant care when participants are hospitalized and communicates participant status to the interdisciplinary team.
