This role is focused on providing both acute and chronic care management for adult and geriatric patients. The APP will work collaboratively with supervising physicians to develop and implement comprehensive treatment plans that improve patient outcomes and quality of life. Foster strong relationships with patients, their families, and the facility staff to enhance care quality. Qualifications: Advanced Practice Provider license (NP or PA). Valid Wisconsin APNP or PA-C License. Valid DEA. BLS certified. Minimum 1year Advance Practice Provider experience (preferred.)
