**Physical Therapist**

Supplemental Health Care is seeking Physical Therapists in Toccoa, Georgia with prior OASIS/Medicare Certified Visits for contract assignments with a partnering home health care provider.

**Job Description**

- $1,940 - $2,126 per week
- 13-week contract
- Paid by the hour, not by the visit
- Physical Therapist will have a designated territory of coverage with a set amount of patients
- Home Health contracts with SHC require previous experience with OASIS/Medicare Certified Visits
