- Attend and contribute to patient care, staffing conferences, and other related meetings.
- Consult with other members of the rehabilitation team to select the most appropriate therapeutic regimen.
- Adapt program and/or plan of care according to the needs of the individual patient.
- Promote a safe environment and enforce the elimination of fire and safety hazards.
- Order supplies and equipment as necessary.
- Clean, maintain, identify, and report the need for repair of equipment as necessary.
- Ensure that patients and families receive the highest quality of service in a caring and compassionate atmosphere.
- Screen, examine, and evaluate patients to establish a diagnosis, identify impairments, determine predicted levels of improvement, and design a plan of care.
- Develop appropriate treatment goals and methods in collaboration with the patient and caregivers.
- Implement the physical therapy treatment plan and complete all related documentation.
- Services include therapeutic techniques such as therapeutic exercise, functional training, manual therapy, airway clearance techniques, health and wellness education, falls assessment, case management in the home and community.
- Design, fabricate, fit, and/or apply adaptive devices; train patients in their use.
- Instruct patients in skills required for participation in mobility activities; evaluate patient progress.
- Perform routine reexaminations as needed/required to modify/progress the plan of treatment.
- Incorporate health and wellness programming into treatment plans for patients.
- Perform home and community mobility assessments; provide recommendations for safe mobility.

**Requirements**

- Must have a valid Physical Therapy license in the state(s) of practice or proof of license eligibility.
- Must have a Master's degree in Physical Therapy or a related field; or a Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience.
- The qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be equivalent to a U.S. degree in Physical Therapy.
- Must have a valid driver's license or reliable transportation to and from patients' residences.
