- MD or DO with Board Certification or Board Eligibility
- Successful completion of an accredited residency (ACGME, AOA, or RCPSC)
- Current ACLS and BLS certification through the American Heart Association
- Comfortable managing a personal patient panel and working independently
- Strong experience in chronic disease management, especially with an elderly population
