Assists developing and implementing the Senior's/client's plan of care in collaboration with the physician, other consulting health care professionals, and/or the family/caregiver. Assists with assuring adequate and effective TEAMember ratios are in place for all shifts. Assist as needed with resident care related activities including but not limited to resident activities of daily living, resident transfers, and other direct resident care tasks as required by the resident and/or clinical teams within the nursing scope of practice.
