**Job Description**

- Provide outpatient rehabilitation services in an in-home setting for the older adult population.
- Evaluate and treat patients in their most functional environment, which may include private residences or independent living facilities.
- Assess the need for, develop, and deliver physical therapy programs to facilitate rehabilitation.
- Identify the need for and make referrals to other disciplines to address the comprehensive needs of patients.
- Implement individualized therapy programs designed to restore, reinforce, and enhance performance.
- Instruct, educate, and train patients and caregivers in skills essential for promoting independence and productivity.
- Direct activities to promote and maintain health.
- Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel as necessary.
- Adhere to department and patient treatment schedules, timely clinical and billing documentation requirements, and attend related meetings.
- Consult with other members of the rehabilitation team to select appropriate therapeutic regimens.
- Adapt programs according to individual patient needs.
- Promote a safe environment and enforce the elimination of fire and safety hazards.
- Order supplies and equipment as necessary and maintain equipment.
- Ensure that patients and families receive the highest quality of service in a caring atmosphere.
- Screen, examine, and evaluate patients to establish diagnoses and design treatment plans.
- Perform therapeutic techniques including therapeutic exercise, manual therapy, airway clearance techniques, and health and wellness education.
