**Clinical Nutrition-Registered Dietitian**

**Job Description**

- Collaborate with providers to identify and connect with patients in need of nutrition counseling
- Administer medical nutrition therapy as part of the health care team
- Establish treatment plans and communicate with patients regarding program compliance
- Make recommendations to primary care providers and assure compliance with standard practices and regulatory requirements
- Document Electronic Health Record documentation with use of the nutrition care process, standardized terminology, and appropriate forms
- Provide evidence-based medical nutrition therapy and implement/update a nutrition plan of care
- Provide nutrition counseling and education to optimize nutritional status
- Screen for malnutrition risk and perform Nutrition Focused Physical Examination (NFPE) when indicated
- Provide appropriate Enteral Nutrition Support using ASPEN guidelines
- Provide appropriate Parenteral Nutrition Support monitoring and recommendations when clinically indicated
- Provide Hostess/Meal Delivery Support
- Indicate when patients are NPO or on Clear diet for > 3 days and monitor appropriately
- Document BMI screenings appropriately for low body weight, extreme obesity, and for advance stroke certification
- Make referrals to appropriate resources and collaborate with other disciplines as indicated
- Perform necessary organizational and administrative duties
- Maintain and complete required reports and statistics

**Requirements**

- Minimum bachelor's degree from a university or college and coursework approved by the Academy of Nutrition and Dietetics (AND)
- Passage of a national examination administered by the Commission on Dietetic Registration (CDR)
- Licensed by the State of Delaware (LDN)
- Previous work experience with medical nutrition therapy in an acute care or long-term care setting preferred
- Must have a current Delaware Dietitian/Nutritionist License

- Able to keep confidential information regarding patients and team members
- Able to withstand crisis situations
- Has skills to provide customer service to patients, team members, and visitors
