**Job Description**

- Family Service League is seeking a full-time Licensed Practical Nurse (LPN) for the Assertive Community Treatment (ACT) program, which provides behavioral health services for adults who have been diagnosed with a severe and persistent mental illness. 
- The ACT program is comprised of a multi-disciplined team including skilled staff in psychiatry, nursing, and social work. 
- The LPN will play an integral part of the team, working under the direction of the Registered Nurse to provide clients with medication management, health assessments, and counseling. 
- The Licensed Practical Nurse will be given an agency vehicle to use for ongoing home visits with the clients. Travel is required.
