- Family Service League is seeking a full-time Social Worker for the Assertive Community Treatment (ACT) program, which provides services to adults who have been diagnosed with a severe and persistent mental illness. 
- The ACT program is comprised of a multi-disciplined team including skilled staff in psychiatry, nursing, and social work. 
- The Social Worker will be given an agency vehicle to use for home visits with the clients. Travel is required.
- The Social Worker will educate the clients and their families about how to manage a severe mental illness and meet their individual goals.
- Conduct daily home visits using the agency vehicle.

- Complete client assessments, relapse prevention plans, and housing applications.
