**Job Description**

- Provide care and services for a group of patients, visiting patients where they reside, which could include private homes, skilled nursing facilities, and assisted living facilities.
- Contribute as a member of the interdisciplinary team, providing expertise regarding psych-social assessments, interventions, and knowledge of community sources.
- Work in either a case management, admissions, or resource role.

**Requirements**

- Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice.
- Ability to work well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
- Strong ability to work closely with patients on a 1-1 level while receiving support from a patient care team.
- Capability to work independently on a day-to-day basis with activities including phone, email and written communication, meetings with various healthcare professionals, and understanding hospice rules and regulations.
