**Job Description**

- Manage a panel of 50-90 patients total; daily load depends on acuity. Some patients are seen monthly and some are seen biweekly.
- Workdays are Monday through Friday from 8 AM to 5 PM.
- Duration of the position is 3 months.
- Focus on a patient population primarily consisting of Medicare recipients.
- Experience needed includes geriatrics, hospice, end-of-life care, chronic disease management, time management, ability to work autonomously, adaptability to change, and flexibility.
