**Advanced Registered Nurse Practitioner (ARNP)**

Seeking experienced ARNP for per diem nursing home services in the Tacoma/Olympia/Silverdale area, approximately 4-6 shifts per month, with the possibility of more in some months.

**Job Description**

- Provide mid-level practitioner services to the geriatric population in sub-acute skilled nursing facility (SNF) stays and residential care.
- Perform in-depth physical assessments and manage illnesses or conditions within the ARNP scope of practice and in collaboration with MDs, pharmacists, and other interdisciplinary team members.
- Diagnose and manage acute episodic and chronic health problems independently.
- Provide comprehensive evaluation and management of acute, episodic, and chronic health problems for patients in skilled nursing facilities and residential care stays, ensuring complete and appropriate assessment and plan of care in a timely manner within ARNP scope of practice.
- Collaborate with attending physicians and provide mid-level practitioner services, including initial evaluation, ongoing medical monitoring, triage of changes in patient condition for the MD/ARNP team, and ensure appropriate medical discharge planning.
- Use prescriptive authority, in compliance with State and Federal law, to appropriately order medications, treatments, and therapies.
- Order and interpret lab work, x-rays, and other tests.
- Facilitate day-to-day plans for ongoing medical care; make telephone encounters or episodic visits as necessary in response to patient, family, and SNF/LTC facility concerns.
- Perform annual physical assessments for residential patients.
- Serve as the primary contact for health and medical care for Kaiser Foundation Health Plan of Washington members during a nursing home stay.
- Monitor quality of care and provide expert clinical resources for nursing home staff.
- Educate patients and families about medical treatment options.
- Participate in family care conferences when needed for health care planning.
- Coordinate care within KFHPW care continuum.
- Ensure complete, accurate, legible, and timely submission of paperwork.
- Attend staff meetings and participate in after-hours call coverage rotation.
- Participate in precepting ARNP students and make presentations to facilities, communities, and colleagues.
- Engage in Nursing Home Services quality improvement projects.

**Requirements**

- Minimum two (2) years of geriatric clinical experience in acute and chronic health problems. (Current KP Coalition employees have this experience requirement waived.)
- Master's degree in nursing from an accredited school of nursing.
- Registered Nurse License (Washington) required at hire OR Compact License: Registered Nurse required at hire.
- Nurse Practitioner License (Washington) required at hire.
- Driver's License (Washington) required at hire.
- Advanced Registered Nurse Practitioner Certificate with Prescriptive Authority (Washington) required at hire.
- Basic Life Support certification required at hire.

**Additional Requirements**

- Geriatric clinical expertise in acute and chronic health problems.

**Preferred Qualifications**

- Two (2) years of geriatric or long-term care experience as a nurse practitioner.
- Expanded prescriptive authority for Schedule II-IV medications.
